Q.  When can I start using my Loyalty Program membership?
A.   Your membership is effective immediately.

Q.  What information is required to sign up?
A.   Simply fill out our enrollment form and include your e-mail address, credit card, full name, address, phone number and date of birth. Credit Card must be on file for monthly payments. However, treatment may be paid with cash/checks.

Q.  What forms of’ payment are accepted?
A.   For your convenience, we accept all forms of credit cards including Visa, MC, Discover, American Express, checks and cash.

Q.  How do I schedule my preventive care (exam/cleaning/digital x-rays)?
A.   Simply call our office and set-up an appointment for yourself and/or your family members.

Q.  How can I use my Member Exclusive discount for restorative (fillings, crowns, etc.), elective or cosmetic treatment?
A.   Simply schedule an appointment with our office and be sure to tell us you’re a member. When you arrive, we’ll make sure your membership is “current” and you will be eligible for instant savings off our regular fees.

Q.  How do I know if need the PerioCare Program?
A.   If you’re a current patient, please contact us to confirm “you receive PM care”. If you are a new patient, we will make a determination after your initial exam.

Q.  What happens it i miss an appointment?
A.   It will be your responsibility to reschedule. Please see our office policy regarding no shows and last minute cancellations.

Q.  Can I transfer my membership to another dentist?
A.   Your Unique Dental Plan membership is only with the office of Dr. Robert Utberg and is non-transferrable.

Q.  How do I nay for treatment?
A.   All payments are made directly to our dental office and are due at the time of service.

Q.  Can i prepay my membership fees?
A.   Yes

Q.  How do cancel my membership?
A.   Complete and sign our membership cancellation form.

Q.  If i cancel, can i get a refund on my membership?
A.   No. Refunds will not be provided for dues paid under any circumstances, including failure to schedule and maintain appointments or moving to another area.
